If you would like to make a referral please contact the Referrals Co-ordinator, on 01604 616454 during office hours (8.30am - 5.00pm, Monday to Friday) who will send you an Referral Form by return.

What happens next?
When you have returned this form to us we will be able to advise on the appropriateness of the referral. If the referral is appropriate we will then request further information to assist us in determining which clinical service is required. Within two weeks we will arrange a face to face pre-admission assessment either at Kemsley or at the patient's current location.
If the referral is not appropriate we will try to assist the referrer to identify a more appropriate resource.
If we accept the patient we will make the necessary funding arrangements after which we will advise on the date of admission.
